1. Any X-RAYS OR STUDIES you have had for this problem.
2. Your health insurance cards including the insured’s (who has the policy) social security and date of birth.
3. Workers compensation claim information or auto insurance claim informaiton.
4. List of current prescription and over the counter medications and supplements.
5. Please bring any copayment required by your insurance.
